The home health agency staff will talk to your doctor about your care and keep your doctor updated about your progress. Doctor's orders are needed to provide home health services.

You and your family will be expected to participate in planning and implementing your plan of care.

Phoebe Home Health accepts Medicare, Medicaid, Private Insurance, VA benefits, and private pay.

If you have a hospital stay, we understand that preparing for a successful transition from the hospital back home is important to one's recovery and to help avoid going back to the hospital. Phoebe Home Health provides a Care Transition program focused on coordination between your health care team to help you and your family understand how to take care of yourself when you get back home.

How often will a Phoebe Home care professional visit my home?

The frequency of home health care visits and services provided are based on your doctor's orders in your personal plan of care. Your doctor may change your plan of care, increasing or decreasing the number of visits or services provided, in order to provide you with the best home health care for your needs.
